About MenoWiser®

About Suzanne Rethans

I’m an investigative journalist, podcast host and founder of MenoWiser. After 25 years of writing for publications including de Volkskrant, Trouw and JAN Magazine, I specialised in women’s healthcare. In 2023, I launched the podcast We zijn toch niet gek? , exploring perimenopausal symptoms and potential solutions.

For years, I’ve been exploring one central question: why are some women completely overwhelmed by perimenopause, while others barely notice it? I’ve spoken to hundreds of women, interviewed national and international experts, and immersed myself in the science behind hormonal, neurological and genetic changes. One thing kept coming up: symptoms being dismissed as psychological, despite having a clear physiological basis.

In 2024, I conducted a DNA analysis involving nearly 200 women experiencing severe mental health symptoms during perimenopause. What did they have in common? The analysis revealed genetic clusters that now form the foundation of MenoWiser. In collaboration with Erasmus MC, the dataset is being prepared for further scientific research. To translate my research into individual guidance, I continued my training in hormone therapy and DNA interpretation and obtained certifications from the Heather Hirsch Academy and iGenius.

Within MenoWiser, I focus on women in early perimenopause who are experiencing increasing symptoms of tension and insomnia and who may primarily need progesterone — something that isn’t currently addressed in Dutch GP guidelines. I also analyse DNA and symptoms using the MenoWiser model.

“The current medical model often falls short in explaining what women are actually experiencing.
With MenoWiser, I want to bridge that gap.”

About Eveline van der Wel, MD

I’m a medical doctor, orthomolecular cPNI therapist and founder of Praktijk Geheel de Mens. With a background in both medicine and business administration, I learned to look at health from a different perspective. In business administration, you learn that every part of a system needs to function properly. Otherwise, the system as a whole can still fail. I applied that same logic to the human body.

What I see time and again in clinical practice are people with complex, unexplained symptoms who get stuck in a medical system designed to look for one cause at a time. In reality, multiple issues are often interacting, and the solution can sometimes lie four layers deeper than expected.

My speciality is DNA analysis and translating genetic information into what is actually happening in the body. Genetic variations in hormone metabolism, neurotransmitters, inflammatory responses and methylation can help explain why two women with the same symptoms may respond completely differently to the same treatment. Within MenoWiser, I bring in this genetic layer, so the recommendations aren’t just based on what works for the average woman, but are tailored specifically to you.

The six clusters
1. Slower methylation
2. Impaired oestrogen metabolism
3. Overactive immune system
4. Dysregulated neurotransmitters
5. Histamine sensitivity
6. Oxidative stress & vascular health

I linked these clusters to an extensive scientific Reference Library containing some of the best available research in this field.

Together, these form the foundation of MenoWiser®.

What this provides
Insight into why your particular pattern of symptoms looks the way it does, which interventions are most likely to fit your profile, and in what order they should be considered.

How MenoWiser came about

As an investigative journalist, I interviewed specialists from across the Netherlands working in this field — from leading gynaecologists and endocrinologists to psychiatrists, immunologists and gut health specialists. This taught me to look across disciplines, something individual specialists, by definition, don’t tend to do, and more and more pieces of the puzzle began to fall into place. Then, when I received the extensive results of the iGene DNA test I had taken myself, I finally understood what had gone wrong for me around the age of forty. That raised an interesting question: what would happen if we compared the results of a large number of women experiencing perimenopausal symptoms? I found an enthusiastic research partner in Eveline van der Wel, MD, iGene’s specialist in DNA interpretation. She shared my belief that telling these women simply to “work on their lifestyle first” didn’t do justice to the complexity of what they were experiencing.

Every woman responds differently to hormonal changes. How your body produces and metabolises oestrogen, how hormones interact with neurotransmitters, how sensitive your stress system is, and how quickly you break down histamine are all influenced to a significant extent by your DNA. What works for one woman may have the opposite effect in another.

After analysing more than 200 women, we identified six clusters. We compared these results with a control group consisting of all DNA profiles in the iGene database. Individually, these clusters were not statistically more prevalent. What was significantly increased was the accumulation of multiple clusters within the same women. The more clusters present, the more severe the dysregulation.

The future of MenoWiser

Available now: personal consultations

MenoWiser functions as an in-depth, personalised neuroendocrine analysis model that unravels complex perimenopausal symptoms prior to a medical consultation. The analysis is conducted by Suzanne Rethans and Eveline van der Wel, MD. Your treating physician can use the analysis as additional information when making a diagnosis and determining treatment.

In development

Clinical decision modules that link symptoms to underlying mechanisms and the appropriate sequence of interventions are currently being validated so they can eventually be implemented in clinical practice. iGene is currently building the prototype for a portal that will bring everything together. In the future, women will be able to log in themselves to gain greater insight into their symptoms. A separate portal is also being developed for prescribers, allowing them to incorporate these cross-disciplinary analyses into their diagnostic and treatment decisions.

The validation process and the CE marking required to enable healthcare professionals to work with the MenoWiser model will require additional time and research. You can help support this work through our crowdfunding campaign.
